Terms and conditions of participation 

GENERAL TERMS AND CONDITIONS OF PARTICIPATION IN THE CONFERENCE 

Chapter 1.
Definitions 

Whenever the following terms should be used in these General Terms and Conditions, they shall be understood as follows: Organiser – Gabinet rehabilitacji Fizjo4life Michał Dachowski, Jagiellońska 4, 03-721 Warszawa, NIP (Tax Identification Number) 1132568602, REGON 145563570
Ordering Party – a natural person not being a consumer within the meaning of Article 221 of the Polish Civil Code, a legal person, or an organisational entity without legal personality concluding a contract for training services with the Organiser in accordance with the principles specified in these General Terms and Conditions and in the declaration of participation in the Conference.
Participant – the Ordering Party being a natural person, or a natural person indicated by the Ordering Party in the declaration of participation, who participates in the Conference.
Conference – a conference, congress, convention, training, or other similar event organised or co-organised by the Organiser.

Chapter 2.
Application 

In order to participate in the Conference, the Ordering Party must send the filled-in application form to the Organiser – electronically as specified at www.ifm-summit.pl

  1. wypełnionego formularza zgłoszenia.
  2. Złożenie formularza zgłoszenia  jest równoznaczne z akceptacją przez Zamawiającego  warunków udziału w Konferencji, wynikających z zamówienia oraz niniejszych  Ogólnych Warunków.

Chapter 3.
Payment and participant registration 

  1. There are the following payment methods:
    1. online payment
    2. payment on the basis of a pro-forma invoice sent to the email address provided by the Ordering Party in the order form
      • Within 2 working days following the submission of the application to the Organiser, the Organiser shall confirm receipt of the application by email and provide the Ordering Party with the information necessary to make payment (bank account number, payment reference and the amount).
  2. Upon the Organiser’s confirmation of receipt of the application, a binding contract shall be concluded between the Ordering Party and the Organiser.
  3. In the case of online payment, the payment shall occur at the moment the order is placed.
  4. The Ordering Party is obliged to make the payment within 7 days of receiving confirmation of the declaration of participation in the Conference from the Organiser. For applications submitted less than 9 days prior to the Conference, the payment must be made no later than 2 days before the Conference begins.
  5. The Organiser shall have the right to withdraw from the contract during the period remaining until the first day of the Conference if the Ordering Party fails to make payment within the period specified in item 3.
  6. The participation in the Conference shall be confirmed after the payment has been registered by the Organiser.
  7. Within 7 days of the payment being registered, the Ordering Party shall receive a VAT invoice. The Ordering Party authorises the Organiser to issue a VAT invoice without the signature of the Ordering Party or a person authorised by the Ordering Party. The invoice shall only be sent by email to the address provided in the application form.
  8. After the participation has been confirmed, but no later than 5 days before the date of the Conference, the Ordering Party shall receive “Detailed information for participants” containing information on the venue, the programme of the Conference, etc.
  9. The Organiser shall have the right to cancel the Conference (withdraw from the contract) during the period remaining until the first day of the Conference if the organisation of the Conference is rendered impossible for reasons beyond the Organiser’s control, e.g. failure to receive the minimum required number of applications.
  10. In the event that the Conference should be cancelled, the Organiser may offer to reschedule the Ordering Party’s participation in the Conference. Should the Organiser fail to suggest a different date for the Conference or should the Ordering Party not accept the date, the amount paid by the Ordering Party to the Organiser’s account shall be returned no later than within 7 working days following the planned date of the Conference to the bank account indicated by the Ordering Party.
  11. In the event that the Conference should be cancelled, the Ordering Party shall not be entitled to a reimbursement of the costs of travel, hotel reservations, and other costs incurred in connection with the participation in the Conference, except for the reimbursement of the Conference participation fee paid to the Organiser.

Chapter 4.
Withdrawal from the Conference

  1. It is required that the Ordering Party’s declaration of withdrawal from the Conference (withdrawal from the contract) be made in writing and delivered to the Organiser by post, fax, or email for it to be valid.
  2. Delivering the written declaration of withdrawal no later than 30 days before the Conference shall place no financial burden on the Ordering Party (date as delivered to the Organiser).
  3. In the event that the written declaration of withdrawal should be delivered 29 to 15 days before the Conference, the Organiser shall be entitled to 20% of the Conference participation fee. If the written declaration of withdrawal is delivered less than 15 days before the Conference or if the Participant fails to arrive at the Conference without submitting such a declaration in advance, the Organiser shall be entitled to the full Conference participation fee. The Ordering Party agrees to bear the above-mentioned costs.
  4. If the Participant is unable to participate in the Conference for reasons beyond the Participant’s control, the Organiser permits the Ordering Party to designate a different Participant. The designation of a different Conference Participant by the Ordering Party shall be made in writing (email) and delivered to the Organiser no later than 4 working days before the Conference.

Chapter 5.
Additional information

  1. Each Participant shall receive a conference package at the Conference venue.
  2. After the Conference, each Participant shall receive a personalised certificate confirming their participation in the Conference.
  3. The Organiser reserves the right to modify the Conference programme or change speakers in cases where the subject matter should so require and when justified by circumstances beyond the Organiser’s control.
  4. The Organiser acknowledges the possibility of organising the Conference on principles different from the ones specified in these General Terms and Conditions. In such a case, the terms and conditions agreed by the Organiser and the Ordering Party shall take precedence, and these General Terms and Conditions shall apply insofar as they do not contradict the said arrangements.
  5. The Organiser declares that the materials shared with the Participants during the Conference are protected by copyright. The Ordering Party and/or Participant can only use them within the scope of permitted personal use. Copying, duplication, distribution, and other forms of material use exceeding the legally permitted scope are forbidden.

 

Chapter 6.
Complaints

  1. Each Ordering Party has the right to file a complaint regarding the Conference within 5 working days following the end of the Conference.
  2. Complaints should be submitted in writing, in the form of a registered letter or email to the address of the Organiser.
  3. Complaints shall be handled within 14 days of being received by the Organiser.
  4. In the event that the complaint procedure should require additional investigation (legal consultation, opinions of external entities), the period of handling a complaint may be extended.
  5. The notification of the complaint settlement shall be made in writing – in the form of a letter or email.
  6. Complaints filed after the time frame specified in item 1 shall not be handled.

 

Chapter 7.
Final provisions

  1. These General Terms and Conditions of Participation in Medical Conferences constitute an integral part of the order in accordance with Article 384 of the Polish Civil Code.
  2. Any disputes arising in connection with agreements concluded on the basis of these General Terms and Conditions shall be settled by the court competent for the registered office of the Organiser.
  3. The Organiser shall be entitled to modify these Regulations at any given time, subject to the rights of the Ordering Party under the version of the General Terms and Conditions which was in force at the moment the Ordering Party received order confirmation being preserved.
